If you are travelling with a child under 18 years old, you need permission from the other parent with parental responsibility. You fill in the consent form together. This shows the Royal Netherlands Marechaussee that you have permission to travel abroad with the child.
Choose your situation:
Do you have sole parental responsibility? Then you do not need permission from the other parent to go abroad with your child. You do need to be able to prove that you can travel with your child without permission.
Proof
You should bring with you:
A recent (recommended: not older than 3 months) international extract from the Personal Records Database (BRP) of the child with parental and custody information. You request this document from the municipality where you live.
Sometimes you cannot get this extract. Then you can take these 3 replacement documents:
International birth certificate or international extract from the BRP of the child with parental information
Recent (recommended: not older than 3 months) extract from the BRP of the parent with responsibility with their marital status
Recent (recommended: not older than 3 months) extract from the Parental Authority Register. This states that the child does not appear in the custody register
Parental responsibility through the court
Did you get parental responsibility through a court ruling? Then bring:
Can you not request a recent (recommended: not older than 3 months) international extract from the BRP of the child with parental and custody information? Then bring:
International birth certificate of your child with a copy of the death certificate of the other parent.
Do you want to travel abroad with your child without the other parent? And do you have joint parental responsibility? Then you need permission from the other parent with responsibility.
Proof
You should bring with you:
Consent form, signed by the parent who is not traveling.
Copy of the passport or identity card of the parent who is not travelling.

No permission from the other parent
Does the other parent with responsibility refuse to give permission? Then you can ask the court for replacement permission. A lawyer is mandatory for this.
Do you want to travel abroad with a child? And do you not have parental responsibility? Then you need permission from both parents who are not travelling.
Copy of the passport or identity card of both parents.
Recent (recommended: not older than 3 months) international extract from the Personal Records Database (BRP) of the child with parental and custody information. You request this document from the municipality where you live.
Instead of an extract from the BRP, you can also bring an international birth certificate of the child.
No permission from the other parent(s)
Can you not get permission from the parent(s)? Then you can ask the court for replacement permission. A lawyer is mandatory for this.
Proving permission with a form
With the completed consent form, you prove to the Marechaussee that you have permission. You sign the form together with the other person who has parental responsibility.
